A patient has transmitted data to the Neutrino portal and is viewable for ordering provider team. - Instructions for taking measurements of cellular blood pressure monitor provided and patient verbalized understanding. This includes: Measure blood pressure with morning readings at least 1 hour after taking medication. Patient was instructed to be rested for at least 5 minutes. Patient should be seated upright with legs and crossed when measuring blood pressure. -The patient was reminded key components of remote patient monitoring program, including checking with the staff or the provider if measurements were out of range Or if the patient is not checking the blood pressure on a daily basis - the care team receives all readings and interpreted all readings as though they were the patient's readings. Patient verbalized understanding that they should not share that device with others. - The patient has access to their measurements and trends and grafts through that patient account on the app which they receive during this set up. - The patient was encouraged to save the Neutrino app on their phone and to call or text us through the in app messages system if they had any questions about the device or the service.
